New York State law requires all students participating in athletics to receive a sports physical completed by their physician. All forms are available at the Athletic Office at New York Ave, the Health Office in the building your child attends or can be downloaded from the School District website (www.smithtown.k12.ny.us go to Departments, then Athletics).
*Health Certificate/Appraisal Form/Physical Form - is to be completed, signed and stamped by your child’s physician. All innocent heart murmurs, allergies, asthma, etc. can be addressed on the front of the physical. Medication orders can be included on the form (i.e. Inhalers, epi-pens). If your child’s physician deems your child capable of administering their own medication he/she can check off the appropriate box and PARENT MUST SIGN THE FORM below the medication order. This physical is good for one calendar year.
*Health Screening/ Medical Update Form- is a one page form that is to be completed and signed by the parent. This form must be handed in with any new physical exam and within 30 days of the first day of the sport season
An email will be sent when the student has been cleared.
