COVID Information for employees

Employees should follow the guidance below if you are required to quarantine (close contact) or isolate (if you've tested positive) related to COVID-19. Employees that have been identified as a "close contact" and are fully vaccinated or previously tested positive for COVID-19 may be exempt from quarantine requirements upon submission of proper documentation. The Suffolk County Department of Health Services provides guidance on the requirements here.

 

Attendance Reporting

Contact your direct supervisor and/or building principal immediately and enter your initial absence into Frontline to secure coverage if applicable. Contact Lynn Clifford (certified staff) for assistance entering an absence of more than 5 days. Non-certified staff that are not familiar with the Frontline system should contact their direct supervisor.

 

At-Home Test Reporting

Be sure to report a positive test result to the Suffolk County Department of Health Services on their reporting portal by clicking here. Please make sure to take a photo of your home test, you will need to supply the test results if you are applying for the NYS Paid Leave.

 

NYS Paid Leave Forms

Review the COVID Updates memo (here) for additional information, the NYS Paid Leave form is linked here. Complete and return as noted within 14 days only if you are applying for the NYS Paid Leave. Submissions that are not timely or are missing required documents will not be processed. Your initial absences will be charged against your sick leave (if applicable) and the time will be replaced upon timely submission and approval of the NYS Paid Leave information.
