The Smithtown Central School District utilizes a centralized registration. All registration takes place at the Joseph M. Barton Building, 26 New York Avenue, Smithtown, NY, 631 382-2024. Registration is open from 8:30 AM to 3:00 PM daily. Listed below are the items necessary to complete registration. Registration in July and August is from 8:30 AM to 2:00 PM.

Parents must bring the following information with them at the time of registration:

1. A child’s original birth certificate (raised seal), a Baptismal record or passport.

2. Your own photo ID, such as a driver's license or passport.

3. A record from physician with all inoculations listed with a doctor’s signature or stamp.

Immunization Guidelines:

3 doses of Diphtheria Toxoid, Tetanus Toxoid and Pertussis (DPT)

3 doses of polio (usually OPV or IPV))

1 dose live measles after age of 12 months

2 doses measles for all children born after January 1, 1985

1 dose live rubella after age of 12 months

1 dose live mumps after age of 12 months

3 doses Hepatitis B for children born on or after 1/1/93

1 dose of Varicella (chickenpox) for children born on or after January 1, 1998 and

6th Graders born after 1/1/1994 entering school on or after 9/1/2005.

1 dose TDAP for children born on or after 1/1/1994.

4. Be prepared to answer questions concerning child’s health history.

5. Two proofs of residency (with your name and address). The most recent utility bills (i.e.home telephone, Cablevision, LIPA, National Grid, water), tax bill, mortgage statement, homeowners' insurance, deed, signed lease including landlord's phone number. Please note: when no lease is in effect, you'll need a notarized statement of residency from the landlord with two original proofs of their residency. The District does not accept driver's license, cell phone bill, auto insurance card, credit card bill, checking or bank account statements as proof of residence.

6. Proof of guardianship or custody, if applicable.
