Clearance Procedure


New York State law requires all students participating in athletics to receive a sports physical completed by their physician. All physicians who administer a physical are required to complete the Smithtown Central School District forms. All forms are available at the Athletic Office at New York Ave, the Health Office in the building your child attends or can be downloaded from the School District website ( go to Departments, then Athletics).

The following must be completed before your child is able to participate in athletics:

  • Health Certificate/Physical Form – both sides are to be completed, signed and stamped by your child’s physician. All innocent heart murmurs, allergies, asthma, etc. can be addressed on the front of the physical.  Medication orders can be included on the form (i.e. Inhalers, epi-pens). If your child’s physician deems your child capable of administering their own medication he/she can check off the appropriate box and PARENT MUST SIGN THE FORM below the medication order. This physical is good for one year.

  • Health History Form – this is a two page form that parents/guardians must complete, sign, date and bring to the physician when your child has their physical. This form and the Health Certificate/Physical form are necessary and must be handed in together to the Health Office.

  • Medical Update Form – this is a one page form that is completed and signed by the parent and student. The form must be completed if your physical is more than 30 days old from the first day of the season. This form must be filled out for each season.

  • Athletic Permission Code of Conduct – this form must be signed by athlete and parent/guardian. The form is given to the coach on the first day of practice.
